What Happens at Your First Physiotherapy Appointment

If you’ve never seen a physiotherapist before, it’s normal to feel a bit unsure about what you’re walking into. Will it hurt? Will you have to get undressed? Will they just give you a sheet of exercises and send you home?

Here’s an honest walkthrough of what a first appointment actually involves, so you know what to expect before you arrive.

We start by listening

The first part of your appointment is a conversation, not a treatment. We’ll ask about the problem that brought you in: where it hurts, when it started, what makes it better or worse, and how it’s affecting your day-to-day life.

We’ll also ask about your wider health, past injuries, your work, and the activities you want to get back to. This isn’t small talk. Someone who needs to lift a toddler has different goals from someone training for a marathon, and the plan should reflect that.

Be as specific as you can. “It’s been sore for a while” is less useful than “it started three weeks ago after a long drive, and it’s worst first thing in the morning.”

Then we assess

Next comes the physical assessment. Depending on your problem, this might involve watching how you move, testing the strength and flexibility of certain areas, and gently examining the painful area.

You’ll usually be asked to move in particular ways so we can see what’s restricted or painful. Some tests may briefly reproduce your symptoms, which is useful information rather than a bad sign. Tell us if anything is too uncomfortable and we’ll adjust.

You may be asked to remove some clothing so the relevant area can be assessed properly, for example rolling up a trouser leg or changing into shorts. You’re welcome to bring suitable clothing with you, and you can ask for a chaperone if you’d prefer.

We explain what we think is going on

This is the part people often say they value most. We’ll tell you, in plain language, what we believe is causing your symptoms and why.

Understanding your problem matters. Pain is a lot less frightening when you know what’s driving it, and people who understand their condition tend to recover better than those left guessing.

If something needs further investigation, or if we think you should see a GP or specialist, we’ll say so.

We agree a plan together

Based on the assessment, we’ll discuss what treatment is likely to help and roughly how long recovery might take. That plan might include hands-on treatment, specific exercises, activity changes, or a combination.

Realistic expectations matter here. Very few problems resolve in a single session, and anyone who promises an instant fix is overselling. What we can usually give you at the first appointment is a clear direction and something to start doing straight away.

You’ll usually start treatment on day one

Most first appointments include some treatment, not just assessment. You’ll typically leave with a small number of specific exercises rather than a long list, because a few exercises done consistently beat many done occasionally.

What to bring

  • Comfortable clothing you can move in
  • Details of any relevant scans, reports, or medications
  • Your insurance details, if you’re claiming
  • Any questions you’ve been meaning to ask

That last one is worth taking seriously. Write your questions down beforehand. It’s easy to forget them once you’re in the room.

A note on nerves
